            When Edward Hospital surgical patient Lillian G. Avery became the first person in the United States to register for Medicare 50 years ago, the Naperville woman gave at least a fleeting thought as to how to parlay that fact into a bit of personal immortality.

              • I've posted previously about The University of Queensland vaccine candidate using novel "molecular clamp" technology. This showed great promise but will not proceed to Stage II/III trials because the vaccine interferes with basic HIV screening tests.

                How does that happen?

                For background, the "molecular clamp" holds the viral spike protein in a fixed shape, preventing it from unravelling as it does during cellular infection. This allows the immune system to identify the circulating virus and deal with it before it infects cells. This gives the best chance for a very effective immune response.

                Now, the clamp requires a suitable protein to make an effective clamp and the UQ team cast a wide net but the only one currently found to be highly functional was a well-studied fragment from the HIV virus.

                While the protein obviously can't give you HIV, using it ran the risk that the immune system would make antibodies to it that would show up in basic HIV screening tests. The UQ team addressed that by removing elements of the protein they thought most likely to be recognised by the immune system.

                Everything OK, then? Well, it worked in the initial animal testing but once in human test subjects, they produced a weakly positive result to HIV screens. Annoyingly, the positive results barely exceeding the test's threshold.

                Now a rich country like Australia with negligible HIV infection rates can just use more discriminatory and expensive testing to bypass this issue altogether but that's not possible in many countries of the world.

                Widespread adoption would require refactoring the common and cheap HIV screening tests in a few months, if that was possible at all. With the emergence of suitable candidate vaccines in UK and USA, they decided there was no point going further.

                The UQ team will continue with the Stage I trial to see if tests all turn negative over time as well as to try and work out exactly what the immune system is responding to.

                They will then need to go back to searching for an effective, non-reactive protein. This probably won't be quick.

                Overall, a disappointing result, especially as the basic technology proved safe and effective across all age groups. Its level of effectiveness gave hope that it would provide sterilising immunity, something not yet apparent in the current leading candidates.

                Maybe the technology will be ready for COVID-23...

                • Australia seems to have stopped community transmission chains after a few hotel quarantine escapes in South Australia and New South Wales. The various jurisdictions are all over two infection periods (28 days) without community transmission apart from those two states and NZ, which will get there tomorrow:

                  ACT 159
                  TAS 214
                  WA 219
                  NT 255
                  NZ 27

                  Internal borders are all open except for those travelling from SA to WA. This will probably be relaxed by the end of the year.

                  Two way travel between Australia and New Zealand without quarantine will recommence in the first quarter of next year. This will be after NZ trials the reorganisation of immigration arrangements to separate non-quarantine incomers from others. It will test this on a smaller scale with travel between the Cook Islands and NZ to come first.

                  If this is successful, and scales up for the much larger Australian market, more Pacific nations are likely to be added to the travel bubble.

                    Who is really ‘first in line’ for the vaccine? It depends on your state.

                    The FDA has authorized the Pfizer vaccine. Health-care workers and nursing home residents could receive the first doses within days, but being in the top tier does not guarantee instant access.

                        • We know that India's death toll is extremely under counted. Their at 145K officially. Then there is this comment from an interesting NYT article on how the virus was spread throughout the country by trainloads of laborers returning home.

                          "that figure (Death Toll) remains a mystery across the country. India has reported far fewer virus deaths per capita than many Western nations, but experts caution that 80 percent of all deaths in the country are not medically certified."
