Apparently in tennis it is OK to be constantly annoying but a violation to only be annoying once:

Did the Hasse let out the mocking grunt when he struck a ball, or was it meant to taunt his opponent when the opponent was hitting?

Players should be held to a standard of sportsmanship, but I think you'd issue a warning for something like this before going as far as altering the score. After all these years, you'd think that they would have sorted out the grunting issue.
